It looks like the motor is 3 phase, so you will need proper machinery to get the motor running, if it even does! Correct it is. 220v, 3 phase, 5hp, 17 amps. It'll probably need replacing and I'll probably go with normal 220 and high amp cabling due to how hard three phase is to find, but if it work...
