The Eastern NY Correctional Facility in Ellenville, NY has this STH-10 on the roof of one of the buildings:

I'm not sure if it's still in use or not, but I do know that the local fire department which is not even half a mile away from the prison also uses an STH-10. I wonder if that could lead to confusion.

At one point in time there was a Soundmaster 115 in Attica, NY. Whether it was for the prison or the community, I do not know.

Auburn, NY Has a 16/16 Sterling M-10 for jailbreaks. It is tested every Saturday at noon along with all other Cayuga County sirens.

I've never seen a siren at Alcatraz. Since it is on an island, there would be no need for public warning. There might be local alarms on the buildings, though.

After doing some digging, I was able to find out that they did have a siren and a steam whistle on Alcatraz. They used to be located on the power house. The siren was later moved to the administration building.

A bit off topic here, but since Alcatraz has been discussed I'll mention it.

I visited Alcatraz during a trip to San Francisco during summer of 2011. The tour is well worth the price; I'd recommend it to anyone visiting the area.

However, it's advisable to book tour date and get ticket on line a couple weeks in advance. The Alcatraz tour is so popular that it sells out a week or more i advance. I was on the first boat to the island the morning I went; sign at ticket booth said there were no tickets available until middle of the next week.

Pretty sure the Chester County Prison here in PA has an xT22 on the roof. I was told they used to test it but not sure if they still do

New Mexico State Penitentiary as of 2005 had/has an STH-10, the 2005 remake of "The Longest Yard" was filmed there, the STH-10 is visible in a few scenes throughout the movie. I think I mentioned this on the board a few years ago, but given the thread topic, it fits in here.

(BIG bump) I was on the bus for Varsity Swim Team and the meet was at a school right next to the Fishkill Correctional Facility (Horrible place for a school, right?) Dallas State Prison in PA has a system of a 1003 and two 2904s which directly replaced said 1003s. Those of you who've been around for a while may remember this photo that Dave Fritz took many aeons ago, of a 1003 on a crazy tower in Chase, PA - that siren is now one of the aforementioned 2904s.
They test every weds. at 12.
EDIT: the 1003 is inactive and was just left in place.
