It looks to me like a regular Model 3/5/7 or whatever that someone got a little creative with as far as metal works go. I've seen chimney caps with that top on them so they could have ripped a cap from one for it.

I found this whilst looking for the Buford Dam water release sirens at Lake Lanier in a Gwinett Times article. The sirens can be heard in the video below along with an image from the article. The sirens sound when water is released from the dam and there are four sirens located between the dam and the GA-20 bridge although exact locations are not known. Here is the article:

UPDATE: From what I have seen, these look to be Altec Lansing Mantaray MR42s with DSA2s behind the large horns. Now just to find them

Wonder why the DSA's there? Seems kinda pointless given the presence of the larger (and likely louder) horns...

Looks like a Model 5 with the bottom half of the housing missing.
Resident of a county with big a mixture of sirens, but in the process of being replaced. :(

By the way, that’s actually a 2903. On Whelens, all layers count as active cells. On similar sirens like Modulators and iForces, the bottom layer doesn’t count.

Here’s a siren I’m unsure of near my house. Haven’t seen or heard this siren go off. It’s located in the back right of the fire dept. if your facing the front. It’s white and looks kinda like a H.O.R. Super Sirex without the 2 cones on it. Anyone got any ideas?
