This one's not in the US, but here in Hungary, this takes the prize of being the lowest mounted siren in all of the country. This one is located at a former Nuclear Bunker and is on a close site. Sadly the siren isn't active anymore.

Federal Signal Model 2 on Fisher Road in Columbus Ohio, This is a siren for the Plant it’s at and isn’t tested, But will go off in the event of a emergency

Here’s another one, W.S. Darley STH-10 in the small town of Knoxville, OH. Goes off for fire calls, would not want to be next to that when it goes off..
