It just happens I am currently sitting at work with a guy who was a Captain or soemthing at Geneva, NY.. he said it is an alert siren that was used for disasters. The main "siren" in Geneva, NY is 2 Gamewell Diaphones mounted on top of the City Hall in downtown (there is also one of those speaker arrays but the speakers are actually mounted to each corner of the bell tower).

Here is a picture of City hall, you can see the Diaphones and one of the speakers at the top of the building. I think there is a second speaker on the opposite corner of the building.

Looks like a fedelcode with a custom housing/horns or something like that..

It looks like one of those Darley sirens. I'm not sure what model though. I think it's called the Darley STH-10, or maybe that's just a fan-name.

I passed this siren today, it is along I-90 east of downtown Cleveland. It does look somewhat like a Model 5/7 with the shroud missing.

Resident of a county with big a mixture of sirens, but in the process of being replaced. :(

Model 5/7's and their Darley/GSC copies always have that triangular bracket for the lifting eye like this one does. I would not like to be the guy that has to go up there to service it!

I inspected one of these at ground level and it had no identifying marks except the Westinghouse motor label. My best guess would be some kind of Sterling or Biersach & Niedermeyer (ACA).
