That would be a diaphone. These are most often used at fire stations to alert firefighters to fire calls, along with noon blasts (depending on the department).

North Brookfield, MA's old Thunderbolt 1003, photographed in August of 2008. The siren was eventually removed in very poor condition. I'm not sure if the fire department either switched to using pagers, or the siren stopped working beyond repair.
