There's only one near Chester, WV.
Location: http://goo.gl/maps/wW6Im
That's the only Equinox siren location I know (for now).

Your link isn't working. This is what I am getting:

502. That’s an error.

The server encountered a temporary error and could not complete your request.
Oh, the link was just working. I'm fixing it now. :/

Here's one in New Carlisle, Ohio: https://goo.gl/maps/drjAk At least, I've heard this is an Equinox.

Yep, that one is definitely an Equinox. Take note of the much noticeably-larger (and fewer) exponential horns at the rear of the projector, as compared to those of a 2001, seen here. Pretty neat, considering Federal didn't make very many of those models prior to the release of the 508. Speaking of the 508, New Carlisle recently installed one on the other side of town earlier this year at the former Westlake Elementary School location (Walsh Drive near Funston Avenue), seen here.

Also, not all of the new sirens in the Beaver Valley NGC EPZ are Equinox sirens. Some are regular 2001-130s, with this one and this one from OhioSirenHunter's map being good examples.
