Looking for siren locations in the USA

I am making a nationwide siren map of the USA and need addresses and street names of siren locations from literally any state. Thanks! :D :D

FS Model 2 on the corner of Red Bud Street and New Hill Road Cassville WV. May/may not be active, there's a cabinet on the pole.
I do dumb things when I;m had asleep.

I have a few I've spotted in my home state, WA.

In Chelan, located at around 212 N Emerson St, sits a 3T22 siren used for forest fires, with no testing whatsoever. Been by this siren a few times, never was unfortunate (or perhaps fortunate) to see it go off. Pretty neat siren to see.

There is another one located in Manson, which I'm 95% sure its a FS Model 5. Used to be mounted just like the 3T22 is, but moved to the corner of the roof on a nearby building, which is around 124 Wapato Way. Its most likely inactive, as it is no longer on its original mount. Surprisingly, I haven't seen anyone put it on any siren map that I've viewed so far.

Oh shoot, you made that map? That's so cool mate, I've been using that map for a while now. Hopefully I can help update the map (noticed the model 5 in Manson wasn't on there despite me seeing it in recent times)

Also yeah, not to sound like a stereotypical siren kiddie, but I grew an emotional attachment to that bolt in particular. First bolt I've ever seen. Not in the best condition to say the least. Poor things been rotting untouched since '93. Just want to give that ol' bolt a fresh coat of paint and a cozy home. Luckily, me and my dad actually got in contact of the SPU, and the chance we're getting it is pretty dang high. Hope to (legally) snatch that bolt soon, just gotta wait for them to finish "investigating" their siren system, which I speculate that most, if not all, false alarms are due to a faulty controller.

Last time I checked, nope. Still sits on the roof of a nearby building. I was there last weekend and I doubt anything changed.
