Wisconsin is packed with ThunderBolts, particulerly older ones, much to my liking. Fond Du Lac county seems to have one of the first 1000Ts built. It sounds great!

2007 was the last year for the Thunderbolts in Franklin County, OH. The earliest models were installed in 1952 and all were removed last year.
There are some Thunderbolts left in smaller communities in Central Ohio. There is one in North Lewisburg, OH, northwest of Columbus.

Alexandria, MN (Douglas Co., MN) has 2 thunderbolts...I think there is some talk about replacing both sirens in '09 but right know that is all it is at this point.

I know of at least four Tbolts in my county. Since I'm in Federal country, there could be many more, but I have not found them yet.
Update: OK, these are the Tbolts I have found.
1 Tbolt in Highlands/Markham IL.
2 Tbolts in Markham IL.
1 Tbolt in Tinley Park IL.
1 Tbolt in Country Club Hills IL.
1 Tbolt in Summit IL.
That's all so far.

The City of Dallas still has TONS of T-bolts. I've only seen 1 or 2 2001's in the city limits, the rest are t-bolts.
