I can contribute for Georgia:
Chatham Co. (Savannah): Used to have thunderbolts but are all gone. One can famously (or infamously) be seen in the intro to Forrest Gump.
Bibb Co (Macon): Used to have thunderbolts, most, if not all are now gone. I'll let Jeb or Ian chime on on Bibb Co. I do know some bits and pieces here and there are left of the old system
Muscogee Co (Columbus): Used to have thunderbolts along with SD-10s. All gone now
DeKalb Co: Used to have thunderbolts, all but one head was removed. The county interestingly only took the siren pieces and left the poles with blower pipes on them, some even still hooked up to power.
Atlanta/Fulton Co: Had some along with Mobil-Directos and one Altec-Lansing setup during the Cold War days. One Thunderbolt is left.
Cobb Co: Had tons of thunderbolts. However. today only 2 are left, both inactive.
Clayton Co: 3 refurbished 1000s in Jonesboro
Upson Co: 6 or 7 (lost track) refurbished Thunderbolts in service
Gwinnett Co: To my understanding Lawrenceville had Thunderbolts at one point because I saw the blower and controls to a Thunderbolt in a Snow Jam '82 news segment that was filmed on Scenic Hwy. near downtown Lawrenceville.
Clarke Co: (Athens): Used to have Thunderbolts. One pole remains
Statesboro (Bulloch Co): Allegedly used to have a system of them. One is left
Robins AFB (Warner Robins, Houston Co.): Has thunderbolts, all decommissioned and replaced with DSAs and modulators
Standing ones are as follows by county:
DeKalb: Oak Grove Elementary School (inactive, blower and controls missing)
Fulton: Richard B. Russell Federal Building (inactive)
Cobb: \Old FS #14 (inactive), Boral Brick Co. (inactive)
Bulloch: Bulloch County Board of Commissions building (inactive)
Upson: Atwater & Day, Yatesville FS, The Rock, Keadle Lumber Co, Hannah's Mill FD, Crest Hwy,
Clayton: Jonesboro Police Station, FS #13, Arnold St.
Houston: Joint STARS road near radar installation, MLK Dr. entrance, Flight control tower, Fire Station #1, hangar near 1st St. and Robins Pkwy., Fire Training Center, Golf Course Rd, March Banks Dr., Robins Theater (Robins AFB, all inactive)
