Unless it was recently reinstalled, the Model 5 in Belle Fourche was removed a few years ago. Heres another Belle Fourche Modulator 44°39'56"N 103°51'01"W. Theres also an I-force in that town, i'll do some digging and try to find it.

I travel through South Dakota frequently, I'll add to this post as I have time.

The siren next to it is a dark green 3V8-H. There is a picture on The Siren Archive.
