No clue if you're marking these, but there is one of those SD-10 intakes on a pole at Martin Luther King and 14th in East St. Louis.
I seems I cant find the SD-10/Mount but ill keep looking.

The Model 5 in Sorento is a 5T. The Fedelcode 5 in Germantown was removed for refurbishing.

Albers has three Thunderbolts, two of which came from BVPS. One of which was Shilohs old 1003. They have an inactive Model 5 across the street from Casey's too. It was the towns original storm siren.

All of the Illinois locations you guys are listing off are also here:

The form link Mosiren1 created doesn't seem to be working, so I'll just post some updates here

1. The inactive allertor 125 in Otterville, Missouri was recently repaired by Table Rock Alerting Systems. My guess as to why? The county was under a tornado warning just over a month ago, and that might of been a wakeup call for the city.
2. Both of the thunderbolts in Gillespie, Illinois are 1000's. Their model 5 is actually a Model 7T.
3. The Thunderbolt 1000T at 37°56'38"N 91°47'14"W (in Rolla, Mo) has been removed. The streetview in the link was taken less than a month ago
One of Indianola, Iowa's old RM-127's is currently sitting in my backyard.

I found this article on the new siren in Centertown MO. From the video it looks like the town had a old Darley siren
