Golva North Dakota also has a Sterling M-5 on a tower right behind the fire hall, not sure if its active though.

FYI: Stettler Thunderbolt in Alberta is getting put back into service as a EAS backup.
Also, not sure if you include removed sirens, but from what people had described when I asked, In Brooks, Alberta, there was once a siren across from the 7-11 on main.

It was described as having horns all around it/360 degrees, also, it never had any unique signals. Apparently, it went off every night for curfew. So I'm guessing an STH-10, basically Right behind the Academy of Learning Career College, Brooks, Alberta. I wouldn't be surprised if it were because High River, Alberta, used to have an STH-10 at the fire hall, which was removed and replaced. A 2001-130 now sits on that same fire hall.
