There are hundreds of letters concerning siren systems and planning on the NRC website. Do a quick search for siren.

That's just pathetic. Purposely bypassing failure detection methods? We're talking public safety here! From that letter it sounds like some big wigs at the plant even knew what was going on but did nothing about it

(Along with ATI, a local radio shop, and a few other people).

Sad, but this happens in other fields as well. One of my other passions is pipeline safety.

10 years ago, Olympic Pipeline Co. had a rupture & gasoline spill in Bellingham, WA, that killed 3 young men. Several issues were involved, but 2 BIG ones that apply to this thread:

1. Their pressure relief valves were set up wrong, so they would not open at the desired pressure. This was compounded when the US CFR required yearly tests of such relief valves was NOT done, but just signed off as being done.

2. The *LIVE* system (SCADA) computer was being used to test new software at the same time as controlling the pipeline. It froze, causing a big delay in detecting the leak. Not illegal, but unwise.

In one NTSB Railroad accident Report, I read where someone had placed tape over the speed limit exceeded horn in a Engine. One of the factors in that accident was speeding!
