If you care to update this, there is a Federal Signal Model 5/7 in Seal Harbor, Mount Desert Island, Maine. It is on the northern part, near the intersection of Jordan Pond Rd & Champlain Rd (ME 3). I've seen this briefly on a trip that I took, but there is no street view enabling me to see it.

EDIT: There's also a diaphone/foghorn on top of Bar Harbor FD and another atop Southwest Harbor FD.
