I am a little confused. The 1983 Federal Signal Corp. brochure lists this eight-speaker rotating EOWS as the "EOWS 812", but everyone else here has called it the "EOWS 816" instead.

Is this just a mistake or did someone discover something regarding the actual model number?
Yes, the EOWS 812 is old and clunky and the overall SiraTone lineup had its faults. They came from a time when Federal was desperate to modernize its aging siren lineup - the Cold War was on its way out, Federal no longer owned the siren market, and none of their products had been seriously innovated since 1959. Meanwhile other siren makers had newer, flashier products to woo over customers.
On the other hand, I distinctly remember seeing the one north of Oshkosh. It looked more reminiscent of a traffic light signal. It was mounted on a shorter pole to accommodate the huge siren head - a T-128 took its place by 2007 and it looks somewhat low mounted in contrast.
I don't remember hearing the EOWS 812 go off, but they would have the same tones as other SiraTone sirens. The only difference would be volume as the EOWS 812 was supposed to be louder. I believe the drivers were divided as the name suggests: 12 drivers for 8 horns, implying not all of them have 4 drivers each.
The EOWS 812 was replaced with the EOWS 612 by 1985. I don't know who got the first EOWS 812 although I'd imagine they all went up within a fairly narrow time-frame.
